Objective:The objective of the present study was to determine if the administration of sildenafil (Viagra) in a clinical setting in men with erectile dysfunction (ED) is associated with a risk of adverse events such as priapism, hypotension, and pulmonary embolism.
Methods:This prospective study was conducted on a group of 40 ED patients with mild-to-moderate ED (defined as male to female ratio ≥ 2.0) who were treated with Viagra (25 mg), sildenafil (25 mg) or placebo. Patients were followed up during 6 months and included in the study.
Results:During the study, 40 patients had a baseline event and 4 had a change from baseline. The patients were followed up with the follow up of 4 to 6 months, and they were able to complete a full medical history and follow up.
Conclusion:This study suggests that men with ED who take Viagra (Viagra) are at a higher risk for adverse events such as hypotension and priapism, which are potentially life-threatening in the context of ED. The combination of sildenafil and Viagra is associated with a low risk of adverse events in men with ED.

IntroductionViagra is a phosphodiesterase type-5 inhibitor (PDE-5i). The recommended dose of Viagra is 50 mg. This drug has been widely used in the treatment of erectile dysfunction (ED). The effects of Viagra have been studied in numerous studies and some of them have shown an increase in blood pressure. In a group of men with ED, 50 mg sildenafil is the recommended dose. This study aimed to assess the clinical efficacy and safety of the administration of 50 mg sildenafil in patients with ED treated with Viagra, sildenafil or placebo in the treatment of ED.
The protocol was approved by the institutional review boards of the University of Basel, Basel and the University of Basel Ethics Committee, and the patients provided written informed consent for the study.
In a group of 40 patients who had been treated with Viagra (Viagra) for ED, the mean change in systolic blood pressure (SBP) was 3.8 mm Hg (1.6 mOsm/cm2) in the group that received Viagra (Viagra) and 4.1 mm Hg (1.6 mOsm/cm2) in the group that did not take Viagra. In the placebo group, the mean change in systolic blood pressure (SBP) was 4.7 mm Hg (1.3 mOsm/cm2) in the group that received placebo and 3.6 mm Hg (1.3 mOsm/cm2) in the group that did not take placebo. In the Viagra group, the mean change in systolic blood pressure (SBP) was 3.6 mm Hg (1.4 mOsm/cm2) in the group that received Viagra and 3.1 mm Hg (1.4 mOsm/cm2) in the group that did not take Viagra. In the sildenafil group, the mean change in systolic blood pressure (SBP) was 3.7 mm Hg (1.2 mOsm/cm2) in the group that received Viagra and 3.7 mm Hg (1.2 mOsm/cm2) in the group that did not take Viagra.
In a group of 40 patients who had been treated with Viagra (Viagra) for ED, the mean change in systolic blood pressure (SBP) was 4.6 mm Hg (1.3 mOsm/cm2) in the group that received Viagra and 2.9 mm Hg (1.2 mOsm/cm2) in the group that did not take Viagra. In the placebo group, the mean change in systolic blood pressure (SBP) was 4.5 mm Hg (1.5 mOsm/cm2) in the group that received placebo and 4.2 mm Hg (1.5 mOsm/cm2) in the group that did not take placebo.
Chloramphenicol
The use of this drug is contraindicated because the use of chloramphenicol will increase the effect of Sildenafil by affecting the patient hepatic and intestinal enzyme metabolism.
Isosorbide dinitrate
The use of this drug is contraindicated because the combination of this drug either increases the effects of the other by vasodilation which leads to fatal hypotension.
Nitroprusside sodium
The use of this drug is contraindicated because the combination of this drug either increases the effects of the others by pharmacodynamic synergism.
Enzalutamide
Use alternative drugs because the use of enzalutamide will decrease the effect of Sildenafil by affecting patient hepatic and intestinal enzyme metabolism.
Acetazolamide
Therapy should be administered with caution because the use of Sildenafil increases the effect of acetazolamide by pharmacodynamic synergism and it also increases the risk of hypotension.
The use of this medicine can lower blood pressure and when combining it with alcohol further increases the effect. You may experience dizziness, lightheadedness, fainting, flushing, headache, and heart palpitation. Also, avoid consuming grapefruit juice because it may increase the effect of avanafil on blood levels.
Cardiovascular disease:
The use of phosphodiesterase-5 enzyme inhibitors is contraindicated in patients with cardiovascular disease. A doctor should consider the vasodilatory effects of this drug and whether they may affect patients with cardiovascular disease. The use of this drug may increase the risk of angina pectoris, AV block, myocardial infarction, ventricular arrhythmia, tachycardia, palpitation, hypotension, postural hypotension, syncope, cerebral thrombosis, cerebrovascular hemorrhage, transient ischemic attack, cardiac arrest, heart failure, and hypertension. These events can occur during or after sexual activity.
Renal dysfunction:
Therapy should be administered with caution in patients with severe renal disease or on renal dialysis. The plasma clearance of this drug is decreased in patients with a severe renal impairment which results in drug accumulation.
Liver diseases:
The use of this drug is not recommended for patients with severe hepatic impairment because the pharmacokinetic disposition of this drug has not been assessed in patients with severe hepatic impairment.
